Complete this form for each Capital Improvement Plan (CIP) request your department would like considered during the upcoming budget process. Submit a separate form for each project, vehicle, major equipment purchase, or other capital need that exceeds a total projected cost of $20,000.

Please provide enough detail for Finance and County Administration to understand what is being requested, why it is needed, the estimated cost, when the funding is needed, and how the estimate was developed.

Routine operating expenses, normal repairs, personnel costs, and recurring departmental expenditures should generally be included in your operating budget rather than submitted through the CIP process.
